Glenoid Innovations in Shoulder Arthroplasty

Activity Description:
Since the introduction of the first glenoid component in 1974, there have been significant advancements in glenoid technology of total shoulder arthroplasty (TSA). Long-term data highlights the procedure’s excellent clinical results when used in the setting of glenohumeral arthritis. Further success has been seen with the development of the reverse TSA procedure for patients with rotator cuff pathology. Although outcomes are generally favorable in shoulder arthroplasty, there are still instances of decreased longevity of the glenoid implant, inferior results, and increased complications. These undesirable outcomes can stem from a variety of scenarios, including glenoid bone loss and arthrosis specifically in young, active patients. This article reviews these scenarios, their current treatment options, and innovations that assist in addressing these difficult deformities. The specific innovations highlighted are the use of augmented glenoid components and baseplates in primary anatomic and reverse TSAs respectively, the alternative center line technique in revision TSAs, and the use of an inlay glenoid component in the younger population. The article also uses a case example of each innovation to demonstrate proof of its success.
